  • Important Information about Proctored Testing

    • Students must make an appointment for testing as seating in the lab is limited and there will possibly be multiple exams occurring on the same day.
    • Please contact your instructor to ask when the test will open and close and what permissible items (calculator, etc.) you will need to bring for the test.
    • The Northwest Houston Center will provide the following items in the testing lab for testing: pencils, scratch paper, hand sanitize, and tissue. Testers will need to bring all other items they need for testing that are permissible for their exam per the instructor's instructions.
    • No student will be able to make an appointment or begin a test before 10:00 am and the testing lab will close at 5:00 pm on all test dates.
    • You will be able to make an appointment for a test date until the day before your test date. After a test date is closed, You will need to choose a different test date.
    • If you are unable to take the exam due to the exam closing, you will need to reschedule your exam with your instructor, the instructor must then submit a new test proctoring form for the exam to the Northwest Houston Center within 72 hours before the exam is administered, and you must make a new appointment.
    • Students registering and taking exams in the Northwest Houston Center agree to adhere to all rules and regulations found in the Testing Services - Academic Integrity Policy.
    • If a student is caught in violation of testing policies and procedures during the administration of an exam, the student's instructor and the Office of Student Conduct will be notified. If you are ban from utilizing the Northwest Houston Center as a testing site because of academic integrity, this will be communicated to the you and your instructor by the Northwest Houston Center and it will be the instructor's responsibility to assist you with taking your exams.
    • PVAMU students with disabilities that need to take an exam for a PVAMU course, must take their exams with the Office of Disability Services. You will need to contact Disability Services at (936) 261-3583/3585 or email them at disabilityservices@pvamu.edu to set up an appointment. This process is only for PVAMU students taking exams for PVAMU courses.

  • If you have been accepted into the University, you are considered a student even if you have not attended any classes. You should have your PVAMU Student Identification Number.
  • NOTE: Your preferred test date is dictated by when your exam opens or closes, choosing a test date that is outside the exam open and close date range will make your appointment form invalid and you will have to resubmit this form.
