Students and employees 18 years of age and older. Students must be enrolled at PVAMU as of November 1, 2021. Employees must be a full- or part-time employee who has not given notice of their resignation before accepting a prize. Contestants who have received a full dosage, which currently is either one or two shots depending on the type of vaccine and manufacturer, of either: (a) a COVID-19 vaccine recognized by the U.S. Centers for Disease Control and Prevention; or (b) a COVID-19 vaccine recognized by the World Health Organization.

Each participant will only receive one entry into the Incentive Program. Duplicate entries will be deleted.
PVAMU will notify each winner by contacting them at the email address provided by the winner on their entry form.
Please contact the site (pharmacy, healthcare entity, local public health authority) where your vaccine was administered.
Winners must acknowledge acceptance of the prize in writing and may be asked to provide proof of identification or other documentation to PVAMU. Failure to do so within ten (10) days of the winner being contacted by PVAMU will result in the winner’s forfeiture of the prize. PVAMU will randomly draw another winner to receive the prize from among the remaining participants.
Up to a $250 credit will be applied to the winner’s parking account at PVAMU to cover the winner’s parking fee in a PVAMU controlled parking lot for 2021-2022. If the employee has already paid for their parking pass, in full or in part, a refund of up to $250 will be issued.
Winners will not be able to convert the prize to cash or any other thing of value.
