Trigger: Cadet enrolls in the Army ROTC classes and wants to pursue a commission

  • Cadet Command Form 137-R: Parental Access to Records
  • DD Form 2005: Medical Privacy Act
  • DD Form 2058: State of Legal Residence
  • DD Form 93: Record of Emergency Data
  • DA Form 3425R: Medical Fitness Form
  • Release / Waiver of Liability
  • Dental Statement
  • College Transcripts
  • Birth Certificate / Naturalization Certificate
  • DD Form 214 / DD Form 220 (If prior service)
  • DD Form 4 series (If current USAR or ARNG service)

Qualification Phase

Trigger: Cadet is enrolled and Care member supports contract / scholarship

  • APFT Card
  • High School Transcripts
  • SAT / ACT Scores
  • Cadet Command form 104-R: Planned Academic Worksheet
  • DODMERB or SMP MEPS Fast Track

Validation Phase

Trigger: Cadet formally offered a scholarship or contract

  • DD 369: Police Record Check (if not already completed by recruiter)
  • SSN Card (copy)
  • Form W-4: Employee’s Withholding Allowance Certificate (for current year)
  • SF 1199: Direct Deposit Form
  • SGLV 8286: Service Members’ Group Life Insurance Election and Certificate
  • Scholarship Award Notification Letter (if applicable)
  • Cadet Command Form 167-R: Scholarship Accept / Decline (if applicable)
  • DA Form 4824-R (Army Reserve) or NGB 594-1R (National Guard)
  • LOA: Letter of Acceptance for UAR GRFD (if applicable)
  • CC Form 227-R: Request for GRFD Non-Scholarship Control number (if applicable)

Contracting Phase

  • Declaration of Intent
  • Region Culture Language
  • SF 86: Security Clearance Worksheet
  • DD Form 4 series: Enlistment Document
