TITLE: ALL-OR-NOTHING: A DNP PROJECT TO IMPROVE UNIARY CATHETER MAINTENANCE BUNDLE DOCUMENTATION COMPLIANCE IN POSTPARTUM

ABSTRACT: Introduction: Urinary catheter maintenance bundle compliance is a key nursing quality indicator closely tied to patient safety. In the postpartum department, inconsistent adherence to the all-or-none bundle elements has contributed to documentation variability, particularly for postoperative cesarean section patients who are at heightened risk for complications when catheter care practices are not consistently applied. Knowledge gaps among registered nurses (RNs) and variation in documentation practices have underscored the need for a structured improvement initiative. The purpose of this quality improvement project is to increase all-or-none urinary catheter maintenance bundle compliance among postpartum RNs caring for postoperative cesarean section patients. The goal is to improve regulatory compliance from 53.8% to 90% through targeted educational interventions, standardized processes, and ongoing performance monitoring. Methodology: A single-group pre- and post-test design, guided by the Plan-Do-Study-Act (PDSA) paradigm will be used to implement and evaluate the intervention. Deidentified retrospective and prospective chart audits will be conducted to assess documentation of the seven required bundle elements. The intervention will include RN focused education, a review of institutional and regulatory expectations, enhancements to electronic health record (EHR) documentation workflows, and structured audit and feedback during leadership rounding. Descriptive and inferential analytics, including paired comparisons of pre- and post-intervention knowledge scores and documentation in the EHR, will be used to evaluate improvements in RN knowledge and documentation compliance over the 12-week project period.
