TITLE: DOES EDUCATION AND TRAINING OF CLINICAL STAFF IMPROVE KNOWLEDGE AND PERCEPTION OF THE MEDICATION RECONCILIATION PROCESS: A QUALITY IMPROVEMENT PROJECT?
ABSTRACT: Introduction: Medication harms approximately one in every 30 patients in healthcare settings. As patients live longer and have multiple morbid conditions, clinicians need the most accurate medication list possible. Medication reconciliation (MedRec) is a comprehensive, multidisciplinary process designed to ensure the accuracy of the patient’s medication list. Medication discrepancies can lead to medication errors and cause adverse drug events. In the primary care setting, the clinical staff are responsible for performing the MedRec process and must possess knowledge and understand the importance of their role in the process. Purpose: The purpose of this QI project is to provide education and training to the clinical staff, to improve their knowledge and perception of the MedRec process to promote patient safety. Methods: A single-group, quantitative, quasi-experimental pre- and post-intervention design will be used to assess change in knowledge and perception of the clinical staff. Training sessions will be mandatory; however, participation in the project will be voluntary. The sessions will be delivered on three dates at the two clinical sites. The identity of the participants will be protected by using letters as the unique identifier. Data will be collected using Qualtrics, an encrypted, cloud-based platform, and exported to Excel for analysis through Intellectus Statistics. The primary outcome will focus on the change in the mean knowledge and perception test scores, using a paired t-test. Minimum estimated sample sizes of 9 for knowledge and 24 for perception are needed to detect a statistically significant change in participants’ scores. Demographic data (e.g., role, length of employment, location) will allow the comparison of variations in mean scores and their influence on score variation. All data will be stored on a password-protected computer and accessible only by the investigator.
