Employment Packets

*All packets must be typed, printed single-sided, and signed by the student and supervisor as indicated on the document.

Student Employment Success Guide – This manual is a guide to becoming and successful student worker or supervisor.  Please review the manual in its entirety.

New Hire Packet – This packet is required for students who have never worked on-campus or been processed through the Student Employment Office.  This packet must be printed single-sided and signed by the student and supervisor.

Rehire Packet – This packet is required for student who have previously worked on campus through the Student Employment Office within the last two semesters.  This packet must be printed single-sided and signed by the student and supervisor.

Student Forms

*All forms must be typed, printed single-sided, and signed by the student and supervisor as indicated on the document.

Employee Personal Data Form – This form provides demographic information as well as a point of contact in case of an emergency on campus.

I-9 Employment Eligibility Form – This form documents that each new employee (both citizen and non-citizen) hired after November 6, 1986 is authorized to work in the United States.

Supervisor Checklist – This form is the supervisor certification checklist for requesting student workers.

Student Employment Application – Complete the student application in detail.  Failure to complete will result in your application being returned, causing a delay or possible disqualification.

W-4 Tax Withholding Form – Complete this form so that the University can withhold the correct federal income tax from your pay.

Disabled Veterans Form – Complete this form stating your status as a disabled veteran in the United States Armed Forces.  If you are not a veteran, please select the appropriate box.

Confidential Release Form -  This form authorizes the University to complete a back ground check.  The University conducts a background check on all potential employees.

Work-Study Guidelines – This form provides general guidelines for workplace learning.  it also describes regulations concerning individuals with HIV/AIDS as required by Texas Health & Safety Code, Chapter 85.  It also ensures that procedures are carefully followed in regards to testing and/or counseling a student or an employee for drug use.

Student Work Schedule – This form displays the student’s work schedule and is completed in a collaborative effort with the supervisor.

Student Employment Statement of Responsibility – This form acknowledges that you understand and will comply with the rules, policies, and regulations as a student worker.

Job Transfer/Request – This form must be completed by the student, current supervisor and new supervisor to transfer.  Job transfer request are granted by the Student Employment Office on a case by case bases and are not guaranteed.
