Adobe Reader is a FREE application that allows you to open and fill the forms from your computer.
|You must have the FREE application, Adobe Reader to view and use these forms.|
|To download the application go to “www.adobe.com” and look for the icon “Adobe Reader”.|
|Follow the instructions to download the application.|
|Once the application has been installed on your computer, the forms become fillable.|
|Go back to the forms and click on the link for the form you want to use, and it should download and automatically open.|
|Move the cursor (hand icon) over each line and if it changes to the flashing text icon “I” you can type information on that line|
|Type in your information|
|Certain area fields will be unfillable (require special attention e.g. signatures)|
|Print out the form and get the necessary signatures before submitting it to DSAL.
Note: The form may be saved and the data changed when you want to use it again. This saves time when you want to use the form again.
