Application for Mid-Management Internship

The following application must be submitted with a completed registration form and a passing score of 80% on the Departmental Practice Exam to be considered for Internship registration.

Step 1. Download the Advisement/Registration and Payment Agreement Form
(Please note: This form must be completed and uploaded (Step 2) before you are
able to fill out the Mid-Management Internship Application)

Please Note: Print and fax a signed copy of the Registration Form to (936) 261-3617

Step 2. The user must upload the Advisement/Registration and Payment Agreement Form
that was completed in Step 1 before proceeding to the Mid-Management Internship Application (Step 3).


UPLOAD THE ADVISEMENT/REGISTRATION AND PAYMENT AGREEMENT FORM
