Making Changes to the Campus Directory

Have you gotten married or changed your name for any reason? Or maybe your name is misspelled, either on the PVAMU Webpage Directory or in the Corporate Directory on the new VoIP phones? If so, you will need to print off the Phone Account Request Form from the PVAMU web site.

Fill out the form with your information. Under the section labeled "Type of Service Request" check the box for Changes. Describe the changes you want made please be specific.

To request a Name Change

  • Print out the Phone Account Request Form
  • Complete all information
  • Obtain proper authorization
  • Fax the Phone Request Form to 936-261-2244
  • Send the original via interoffice mail to: ITS, RM 126, S.R. Collins.

Directories will be updated every Friday.
