Adobe Reader

To download the application go to "www.adobe.com" and look for the icon "Adobe Reader".

Follow the instructions to download the application.

Once the application has been installed on your computer, the forms become fillable.

Go back to the forms and click on the link for the form you want to use, and it should download and automatically open.

Move the cursor (hand icon) over each line and if it changes to the flashing text icon "I" you can type information on that line

Type in your information

Certain area fields will be unfillable (require special attention e.g. signatures)

Print out the form and get the neccessary signatures before submitting it to DSAL.
Note: The form may be saved and the data changed when you want to use it again. This saves time when you want to use the form again.
