Workers’ Compensation

Workers’ Compensation is a state regulated insurance program that provides reasonable and necessary medical coverage and indemnity payments to employees who sustain injuries or occupational disease while in the course and scope of their employment.

General Responsibilities

Any employee who sustains an injury on the job must notify their supervisor immediately. The First Report of Injury (DWC Form 1) cannot be completed solely by the employee. The form must be completed in its entirety by the supervisor, along with the employee, and turned in to the Office of Human Resources immediately. The employee will also need to complete The “Request for Paid Leave” form and return it to the Office of Human Resources. Click here for all available Workers’ Compensation forms.
